- You're 18 or older. We don't see minors at this time.
- You live in or can travel to Virginia, Utah, Georgia, or Texas.
- You have a phone, laptop, or tablet for telehealth, or you can drive to a clinic.
- You have a photo ID and an insurance card, or you can pay self-pay rates.
What to bring
Have these ready for your first visit.
- Photo ID (driver's license or state ID).
- Insurance card, both sides if applicable.
- A list of medications you're currently taking, including dose.
- Any past records, imaging, or labs you want your clinician to see.

Either works. Most evaluations can happen by telehealth from your phone or laptop. If a state law or your insurance requires an in-person visit, we'll book you at the clinic closest to you.
Photo ID, your insurance card, and a list of any medications you currently take. If you have past records or imaging, send them ahead through the patient portal so your clinician can review before you arrive.
Yes. You can bring a partner, parent, or friend to any appointment. For telehealth, they can sit next to you on the call.
A full intake with one of our clinicians. We talk through your history, what brought you in, and what you've already tried. You leave with a written plan and, when appropriate, a prescription or a follow-up scheduled.
